Documentation needed to apply for Free Care:

    1. Proof of Income Ž
    (Example)
    · Two consecutive weeks' worth of recent pay stubs
    · Letter from the employer (with company name, name and signature of your boss, how many hours you work, and your total gross; salary paid)
    · Most recent tax return (if self-employed).

     

    · Proof If you receive any other benefits or support:
    (Example)
    A. Letter from Social Security or SSI (award letter)
    B. Child support documentation
    C. Unemployment stubs
    D. If you are unemployed and do not have any income, we will need a letter from the person who is financially supporting you.
    2. Assets (Over 65 applicants only)
    (Example) 
            · Bank checking or saving monthly statement
            · Credit union monthly statement
            · Life insurance monthly statement
